Imaging procedures are essential for the stereotactic radiotherapy of brain metastases. Precision radiotherapy significantly improves the success of treatment. Imaging techniques are already used for this purpose in the diagnosis and planning of stereotactic radiotherapy of brain metastases. Different methods, such as magnetic resonance imaging and computed tomography, are used. Shortly before treatment, additional image guidance acquisition is performed with the help of a Cone beam CT. This allows the immediate correction of deviations in positioning and facilitates modern precision radiotherapy. Stereotactic radiotherapy of brain metastases in particular takes place within highly sensitive risk structures and must therefore be planned and performed with pinpoint precision to minimize side effects for the benefit of the patient.

Combined planning of brain mets saves time and reduces radiotherapy side effects
CBCT imaging in radiotherapy should be used whenever the type of tumor, the tumor location, the calculated dose, or adjacent risk structures limit safety margins. Radiotherapy for brain cancer is most often performed in high-risk tissue, such as near the brain stem or optic nerve. With CBCT imaging in radiotherapy, the patient’s treatment risk is reduced despite the overall higher exposure to radiation. Imaging and fixation with face masks for brain tumor radiotherapy improves the radiotherapy’s precision. At the same time, the duration of treatment can usually be shortened because immobilization with the face mask for radiotherapy allows higher and thus more efficient doses in the target structure.

What is image-guided radiotherapy? Its most distinguishing feature is the tremendous precision of image-guided radiotherapy. Linear accelerators are equipped with computed tomography image guidance.

Stereotactic radiotherapy is usually performed in six steps, which also apply for brain tumor radiotherapy:

Imaging techniques are continuously improving the quality of brain tumor radiotherapy. Even in the planning stage of stereotactic radiotherapy, images from MRI and CT are used to precisely determine the position of the tumor in the patient for radiation. The radiation plan specifies the contouring of the target structure and the doses and frequency of radiation.
A dummy can then be used to simulate and verify the radiotherapy. This is followed by the actual treatment of the immobilized patient in accordance with the established parameters. If the radiotherapy machine images are then evaluated before the radiation treatment and the patient is corrected with the help of a 6D couch, the surrounding tissue is spared as much as possible. The success of the treatment is monitored during follow-up care.

The procedure supports the correct positioning of the patient, especially for high-precision, fractionated stereotactic radiotherapy. For small-volume tumors especially, fractionated stereotactic radiotherapy is an efficient procedure that allows the surrounding tissue to be spared as much as possible. For this reason, it is also used for brain metastases. A mask for radiotherapy is used to ensure that the patient’s head remains absolutely immobile during high-precision radiotherapy.
The mask for the thermoplastic shell radiotherapy of brain metastases is usually a thermoplastic shell that is customized to fit the patient’s face and head. The thermoplastic shell is heated for this purpose so that the material can be flexibly applied to the contours of the face. After cooling, the thermoplastic tray for the radiotherapy of brain metastases is rigid and reliably secures the head to the treatment couch.
